Referrals & Authorization

As a patient at this facility it is your responsibility to make sure that all referrals, authorization for treatment, and prescriptions are both valid and good for each date of service or treatment.
Prescriptions

All prescriptions must be dated, prescribed by a specialist (i.e. orthopedist, neurologist, physiatrist, etc.), must clearly specify a clinical diagnosis (your specific injury and its location right or left), treatment plan, duration, and frequency.
30-day Rule

As a safety precaution, as well as many insurance requirements, it is this facility’s policy that you must return to your treating physician every 30 days for an updated prescription.
Time of Service

When arriving for your visit, please make sure to sign in at the front desk, inform the receptionists that you have arrived, and notify us of any changes to your insurance coverage, personal information (i.e. address, phone), and any change in health.
Co-payment / Co-insurance

All commercial, managed care, and HMO insurance plans are subject to either a co-payment and / or coinsurance. All co-payments are due prior to beginning your therapy session. Any co-payment not paid on or before the scheduled appointment will be subject to a $5.00 collection fee per co-payment.
Waiting for your appointment

Patients are asked to please be patient while waiting for their scheduled appointments. You will be notified as to when you can go into the treatment room. Please do not head to the back on your own. When your therapist is ready and space is available, you will be called back. Patients who begin their treatment with exercise may be called earlier than those patients who need the usage of specialized equipment such as an exam table, electrical stimulator, and / or ultrasound unit. At no time may you be in a treatment area without a therapist or a therapy technician present.
Dress Attire

As a patient, you may be required to exercise on our gym equipment, therefore all patients are required to wear sneakers, running shoes, or walking shoes for protection during all visits. Wearing appropriate clothing that easily exposes the body part(s) being treated will assist in facilitating the therapist ability to treat properly (Examples include: Shoulder = tank top, hip / knee = shorts). Loose fitting attire that you would exercise in is recommended. If for any reason you are uncomfortable during manual treatment, simply inform your therapist, and you will be placed on a curtained off table.
Making Appointments

Following your initial evaluation, if you have not already done so, be sure to make appointments for the specified number of times per week, as directed by your physician, your therapist, as well as allowed by your insurance. If you wish to schedule with a specific clinician, please inform the front desk. We ask that you try to manage appointments between no more than two therapists, so as to maximize continuity in your progression. Please remember that the appointment schedule is as flexible as your calendar allows you to be.
Notifying your Therapist

Your treatment is never finished until you speak with your therapist. If for any reason you must leave early, please notify one of the therapy staff members prior to doing so.
Cancellation Policy

Please note if you are unable to keep your scheduled therapy appointments, please make sure that you have notified the front desk of the cancellation, and or rescheduling of the appointment. You must cancel an appointment 24 hours in advance. All appointments cancelled on the same day will be subject to a $10 fee. Patients who do not call and cancel their appointment will be subject to a fee of $25.00 for a physical therapy appointment, $50 for an initial evaluation, $50 for a massage / acupuncture / PENS treatment. Please make sure to remember to receive a printed schedule from the front desk before leaving.
Re-evaluation / Re-assessment

Please make sure you remember to schedule your re-assessments / re-evaluations on a regular basis, prior to any authorization expiring. We would like to maintain continuous treatment and try to avoid any breaks in care due to obtaining authorization from your insurance carrier. Please remember that the insurance carriers sometimes take anywhere from 48-72 hours before authorizing treatment.
At Our Discretion

It is at the discretion of your treating therapist whether or not to treat you on the day of your appointment. On the basis of the insurance, prescription, and or health condition. At no time will your therapist treat you if you are under the influence of alcohol or narcotics.
Insurance Eligibility and Verification of Coverage

As a courtesy, the staff at this facility will verify your insurance coverage for your therapy benefits. This will include verification of eligibility, number of visits per condition and / or for the year, co-payment, co-insurance, deductibles, in or out of network service, and authorization requirements. As a reminder the information we receive is not a guarantee of your coverage. If for any reason your insurance is cancelled, terminated, or the policy changes (including co-payment) you will be responsible for the cost of treatment. Please make sure you check with your carrier on a regular basis.
One Treatment per Day

Unfortunately, due to many insurances not allowing multiple services being performed on the same date, we have implemented that only one method of treatment can be done on each date of service, unless otherwise noted by your insurance carrier. Therefore, you may be treated for physical therapy, massage, acupuncture, or PENS. If you see our physician for orthopedic services, the appointment must be scheduled on an alternate date.
No-Fault / Work-Comp

If your injury is related to either a motor vehicle or work related accident, you are responsible for notifying the staff immediately. If you are a no-fault or worker’s compensation patient you may only be seen by one specialist per day, this includes outside physicians for the injuries related to your accident. Please see our information regarding no-fault and workers’ compensation.
